Forms, consent and age checks
Health questions, waivers and minimum ages, and how clients sign them.
Making a form
Go to Settings → Forms. Start from a template (for example tattoo, piercing, waxing or lash extension consent), or from scratch. Templates are starting points, not legal advice: check the wording before you use them.
A form can have headings, wording clients read, questions (text, pick one, pick several, date of birth) and an "I agree" tick. Save it, then attach it to services on each service's page under Forms and age, where you also choose how often clients fill it in.
When clients fill it in
After booking, clients see Fill in your forms on their booking and in their email. At the front desk you can also choose Fill in on this device and hand your tablet or phone to the client. Clients sign with a finger or mouse.
Signed forms
Each signed form is kept on the client's record, can be printed or saved as a PDF, and can't be changed. Editing a form makes a new version; what people already signed stays as it was. To cancel a mistaken one, Void it with a reason. It stays on record, marked void, and the client signs again next time.
Minimum age
Set a minimum age on a service. Clients give their date of birth when they book, and you check photo ID in person at check-in (nothing is uploaded). Optionally, younger clients can book with a parent or guardian's consent on the form, where the law allows it.
